I have a few questions regarding light visibility and falloff in Maya. First, how do you make quad lights visible, like the one depicted in the documentation?

You cannot make quad lights visible (not yet, at least). You could create an emissive geo with the right visibility settings (visible to camera rays and specular reflection only).
You could also turn a poly plane into a mesh light, which has a visible option.
